Introducing ZEDOC's digital care pathway library - a fast and affordable way to measure what matters most to patients

Last year, we published an eBook looking at the digitisation of ‘care pathways’, which describe the stepwise movement of patients through the healthcare system. Digital care pathways consist of digital interactions between providers in the hospital or clinic and patients at home, supporting the collection of health outcomes, experiences and delivery of educational content.

These care pathways are aligned with Michael Porter’s concept of Value Based Health Care. By enabling the exchange of important healthcare information to occur digitally and remotely using mobile devices that patients already own, digital care pathways have the potential to transform healthcare delivery.

However, in conversations with healthcare providers across the globe, we often see a few key challenges get in the way of digital care pathway implementation. Those challenges include:

  • Not knowing where to start or what information to collect

  • Time to implement

Leveraging our experience in the development and implementation of digital care pathways, particularly patient-reported outcome measure (PROM) and remote patient monitoring (RPM) programs, we set out to provide care teams with a simpler, faster and more affordable solution to get them started.

What is the ZEDOC Digital Care Pathway Library?

The ZEDOC digital care pathway library is a suite of off-the-shelf and validated digital care pathways that automate the collection and analysis of key health information, particularly PROM data.

Each pathway, which is specific to a condition or treatment, contains a predefined and evidence-based schedule of digital interactions with patients. These evidence-based schedules have been developed and validated by the likes of the International Consortium of Health Outcomes Measurement (ICHOM) as well as researchers and clinical groups.

At each touchpoint in the schedule, ZEDOC automatically communicates with patients and shares secure links for them to complete their latest health assessments on their own devices, at home. Importantly, care teams can customise their own messaging and branding within their team’s care pathway.

With the high quality health outcomes data fed back to care teams in real-time, our digital care pathway solutions give clinical teams the ability to deliver more personalised, proactive and outcomes-driven care.
